By submitting this form and expressing interest in our Feather Touch Brow Model Offer, you agree to the following terms and conditions:

1. Eligibility

Applicants must be 18 years or older.
This offer is designed for individuals who meet the medical, aesthetic, and procedural suitability required for cosmetic tattooing.
All applications are subject to review by our team. We reserve the right to decline participation if we believe it is not in the applicant’s best interest (e.g. skin type, medical history, previous tattooing, or contraindications).

2. Model Offer Inclusions

The Feather Touch Brow Model Offer includes:
- One full Feather Touch Brow treatment
- Pre-treatment consultation
- Custom brow mapping and colour selection
- Healed aftercare plan
The total cost is $250 AUD, significantly reduced from the regular price of $650 AUD per session.

3. Model Agreement

By participating in this offer, you agree to allow Smooth Beauty and Styling to:
- Take high-quality before, during, and after photographs of your treatment
- Use your imagery and/or video footage for marketing, advertising, social media, and training purposes
Your name and any identifying information will not be disclosed without your explicit permission.

4. Booking & Payment

A non-refundable booking fee of [$xx] may be required to secure your spot. The remaining balance is due on the day of treatment.
Rescheduling must be made at least 48 hours in advance. Late cancellations or no-shows may forfeit your model offer.

5. Results & Aftercare

Individual results vary depending on skin type, healing process, lifestyle, and adherence to aftercare instructions.
A touch-up session is not included in the model price but may be recommended at an additional cost.
Aftercare instructions will be provided and must be followed to achieve optimal results.

6. Medical Disclosure

By proceeding, you confirm that you have disclosed any known allergies, medical conditions, or previous brow tattooing procedures.
If you are pregnant, breastfeeding, undergoing chemotherapy, or have any immune or blood-related conditions, you may not be eligible.

7. Limitation of Liability

While every effort is made to provide a hygienically, safe environment with professional, high-quality results, outcomes from cosmetic tattooing can vary depending on individual skin type, healing ability, lifestyle factors, and adherence to aftercare instructions.

By proceeding with the treatment, you acknowledge and accept that:

Final results may differ from reference images and initial expectations.
Fading, colour changes, or pigment retention issues are possible and may require additional sessions at your expense.
Our artists are trained and experienced, but exact results can never be guaranteed.
You agree that Smooth Beauty and Styling and its team are not responsible for outcomes that result from factors outside of our control, including (but not limited to) previous cosmetic work, skin conditions, or improper aftercare.
By signing up, you release Smooth Beauty and Styling from any liability for dissatisfaction, allergic reactions, or complications arising from the treatment.
